Birth and Death Records

The Office of Vital Statistics maintains birth and death records for the City of Portsmouth, with records going back as far as 1909.

Birth records are important for use in entrance to school, military service, obtaining passports, retirement pensions and social security pensions. Death records are needed for settlement of insurance claims, settlements of estates, pensions and tracing of ancestors.

Birth certificates are open records and may be purchased at the Portsmouth City Health Department as long as the person was born in the state of Ohio.

There are two ways in which to obtain a birth or death record in Ohio.

Visit Our Office

Same day service is available to walk-in customers. Please complete the attached application or you may complete an application when you arrive and pay the $25.00 required fee.

Payment may be made by cash or check with a picture ID only.

NO DEBIT/CREDIT CARDS ACCEPTED.

Via US Mail

Turnaround time is estimated to be 1 to 2 weeks from the day the request is received.

You must mail a money order of $25.00 payable to Portsmouth City Health Department for each certified birth or death certificate.

Cash or personal checks are not accepted through mail request.

Please enclose unsealed, self-addressed stamped envelope along with the completed application (attachment below), is required and should be mailed to:
